{"id":10530,"date":"2026-06-18T09:12:53","date_gmt":"2026-06-18T08:12:53","guid":{"rendered":"https:\/\/chimar.eu\/near-miss\/"},"modified":"2026-06-22T09:11:41","modified_gmt":"2026-06-22T08:11:41","slug":"near-miss","status":"publish","type":"page","link":"https:\/\/chimar.eu\/en\/near-miss\/","title":{"rendered":"Near Miss Reporting Form"},"content":{"rendered":"<div class=\"wpb-content-wrapper\"><p>[vc_row][vc_column][vc_row_inner padding_bottom_multiplier=&#8221;2x&#8221;][vc_column_inner][vc_column_text css=&#8221;&#8221;]<\/p>\n<h1>Near Miss Reporting Form<\/h1>\n<p><span style=\"font-weight: 400;\">Italian Decree-Law No. 159\/2025 has made it mandatory for companies (with more than 15 employees) to track and analyse  <\/span><b>near misses<\/b> \u2013<span style=\"font-weight: 400;\"> that is, sudden incidents that could have caused damage or injury but which, due to fortuitous circumstances, did not result in any consequences.<\/span> <\/p>\n<p><span style=\"font-weight: 400;\"><strong>LESI<\/strong>: Worker Reporting a Near Miss<\/span><br \/>\n<span style=\"font-weight: 400;\"><strong>GRTVI<\/strong>: Incident Reporting, Transmission and Assessment Group <br \/><\/span><span style=\"font-weight: 400;\"><strong>GRI<\/strong>: Incident Resolution Group. This may include the GRTVI and\/or the DL. It assesses and closes the incident if it cannot be resolved within the department.  <\/span><\/p>\n<p><span style=\"font-weight: 400;\">The report submitted via LESI will be handled by GRTVI, which
