Near Miss Reporting Form

Italian Decree-Law No. 159/2025 has made it mandatory for companies (with more than 15 employees) to track and analyse near misses – that is, sudden incidents that could have caused damage or injury but which, due to fortuitous circumstances, did not result in any consequences.

LESI: Worker Reporting a Near Miss
GRTVI: Incident Reporting, Transmission and Assessment Group
GRI: Incident Resolution Group. This may include the GRTVI and/or the DL. It assesses and closes the incident if it cannot be resolved within the department.

The report submitted via LESI will be handled by GRTVI, which will interact with GRI to resolve the issue and provide feedback.
